Job application forms: Go to

Transportation: Use the
Bus Stop Change Form for 2010 to request that
your child be regularly transported to a
babysitter's home.
School / Sports Health Exam
- for your physician to complete when your
child has a physical (Updated May 2008).
Sports Health Recertification Form - for
parents to use ONLY if their child's
physical was 30 or more days before the date
of sports tryouts.

Facilities Use: Complete the
BH-BL Facilities Use Form to request
that your organization be allowed to use any
school district facility (indoor or outdoor)
for its event.
School Handbooks These handbooks are written for parents and students at each
school. They describe daily procedures and
policies
including the Code of Conduct.
Guidelines for Participation in Interscholastic
Sports for 2008-09 (Note: This
document was designed to be printed on legal
